NYLA's Office of Intellectual Freedom is encouraging any librarian to alert the office to challenges. This form is being used to collect data and will not be used to generate a response. Information submitted using this form should be considered public and may be subject to FOIL requests, so please limit or omit any personally identifiable information when you fill it out. As always, should you face a challenge, keep CALM and CALL (don't email) your SLS Director.
