Youth and Adult Leader Forms

You can download the youth or adult application from the Scouting America Southeast Louisiana Council links below. Once completed, the forms can be returned to the leader of Troop 115 to join.

Scouting America Youth and Adult Membership Applications

All Adult Applicants must complete S.A.F.E. Guarding Youth Training (Formerly Youth Protection Training) at My.Scouting.org before your application can be processed.

Please print and include the Youth Protection Certificate with your Application or you can e-mail the training completion certificate to srstroop115@gmail.com.

Medical/Health Forms

If a youth and/or a Family member plan to attend overnight camping trips or events, then a medical Form needs to be filled out and submitted to our Troop annually. The medical form can be downloaded, printed and filled out from the Scouting America Southeast Louisiana Council link below.

Scouting America Health and Medical Records Form

Part "A" and Part "B" can be filled out be a adult, parent, or legal guardian. However, Part "C" needs to be filled out by a Physician.

Once you are on the Scouting America Health and Medical record Forms website, please click the download link under the "Are You Going To Camp?" heading. This is the correct form that needs to be downloaded, printed, filled out and submitted annually to an Adult Leader in our Troop in order for a youth or adult to attend any overnight/camping activities with our Troop.
