All students are required by law to be up to date on their immunizations before they start class. The Illinois State Board of Health will be checking health files to be sure all students are in compliance with the state health law. Forms are also available on this page, in the school office, or through the Illinois State Board of Education website, www.isbe.net. You can have your health care providers fax the form directly to us at 312.226.7265. Please find below a chart that breaks down the grades and the required forms needed. We recommend you make an appointment with your child’s pediatrician now. During registration follow up days, parents are required to turn in health forms or present an appointment date.
Students will not be allowed to start classes if examinations are not on file. No exceptions.

Required documents to participate in athletic programs
All students wishing to participate in extracurricular sports programs must carry health insurance. Proof of insurance must be presented along with a current physical examination, a signed concussion acknowledgement and consent form, and the sports contract. These documents can be found on the parent portal under Sports Documents.
If your child is interested in participating in any of our intramural sports activities, your child will need to have the following documents on file:

Have your physician fill out the athletic form or check the box on the annual physical document.
