Classroom Observation Form- For completing classroom observations as part of an evaluation.
Determination of Eligibility/ Eval Sum Meeting- This document has information and checklists for the evaluation summary meeting and determination of eligibility meeting information.
Guiding Questions for Referrals This document provides a guide for discussion with teachers and parents at referral meetings.
Guidelines for Determination of Eligibility- This document is intended to be used to determine if a student qualifies for special education services. Please use the specific disability category decision making trees to determine under what specific category the student qualifies for services under.
Email Template- This email template is intended to be sent to teachers to schedule a date and time for an observation. At some schools, connecting with teachers in person to schedule is the norm. If that is the case, you would not need to send an email.
Evaluation Summary Meeting Agenda- This document has a checklist for paperwork and meeting requirements.

KTEA-3 - Determining Subtests to Administer - The entire KTEA-3 does not always need to be administered. This document helps teachers determine the appropriate subtests based on the referral or reevaluation.

Re-Evaluation- This guide has information about the re-evaluation process. This process is similar to the initial evaluation process and is completed every 3 years.
