Special Education Required Forms, Timelines, and Procedural Manuals
Maine's DOE published special education forms for all districts to use. In New Hampshire, ask your special education administrator for district-specific forms.
Here are the special education timelines in a brief, 1-2 page handout:
Academic Progress Note Examples- These are examples of progress notes based on different goals and areas of instruction.
Advocate Procedure- This procedure is designed to be used when an advocate joins any meeting throughout the special education process.
Case Studies- Case studies for a referral moving forward and a referral not moving forward.
Guiding Questions and Record for Teacher Consultation- Note when you meet with other educators and their responses to the questions listed on the form for reference.
Informed Consent (NH) - This document outlines instances when the team needs caregiver consent.
IEP Meeting Agenda - This document helps the case manager organize the IEP meeting.
Meeting Minutes Quick Guide (NH Only)- A short guide to writing meeting minutes in New Hampshire. Maine does not need to write meeting minutes as the WPN serves as the minutes.
Procedural Safeguards Handbooks
Procedural Safeguards, or Parental Rights, are to be offered to parents/guardians at least 1 x school year. Depending on the state, Procedural Safeguards are also provided at additional specific times (e.g. initial referral, manifestation meeting, DOE complaint, etc.).
Ready for Dismissal- Guidelines for determining when a student is ready for dismissal from special education services.
Recording Request- This document is intended to serve as a guideline for how to proceed when a family or team member requests that a meeting be recorded. If someone requests this, immediately notify your SDM and special education administrator or school-based contact.
Responding to Family Concerns- It is critical to maintain open lines of communication with families and caregivers throughout the entire special education process. You are the expert in the process, but the families/caregivers are experts in their child.
Request for Pre-Referral Academic Screening- This form is intended to be used before going through the referral and evaluation process.
Request for Independent Educational Evaluation - Consult your coordinator or director and SDM! This document provides step-by-step guidance for responding to a family/caregiver formal request for an Independent Educational Evaluation (IEE) at public expense.

Written Prior Notice Quick Guide- Written Prior Notice (WPN) is a formal, detailed document that outlines any actions the district is proposing or refusing related to a student's special education services. It also includes the rationale behind those actions, as well as the factors considered in making the decision.
