3 Administration Formats:
Interviewer Form: semi-structured interview timed to obtain information about the interviewees adaptive skills/functioning from a parent/caregiver
Parent/Caregiver Form: questions about family and home life completed by parent or caregiver
Teacher Form: gathers information from teachers (present and past) about an individual's adaptive behavior in school
Individual's behaviors are rated on a scale of 0 (never performed), 1 (sometimes performed) and 2 (typically performed)
