Teacher Rating Scale: different forms focus on age appropriate behaviors (age 2-5, 6-11, 12-21). Rate behaviors never, sometimes, often, almost always.
externalizing problems, internalizing problems, school problems, adaptive skills composite, behavioral symptoms index
Parent Rating Scale: same as teacher rating scale but for home behaviors
same as above minus study skills subscales, added activities of daily living
Self-Report Personality: yes/no questions with follow-up questions for ages 6-7, other versions mark short statements true/false or rank from never to almost always for ages 8-11, 12-21, and 18-25

Structured Development History: interview or questionnaire outlines social, psychological, developmental, educational and medical history
Student Observation System: adaptive and maladaptive behaviors coded on paper or online in a 15 minute observation

time sampling of behavior: was behavior present during a 30-second observation?
teacher interaction: interactions with students coded on position, techniques used to respond to behavior, additional relevant observations
Measures the occurrence of adaptive and maladaptive behaviors which represent both behavioral problems and strengths including: internalizing problems, externalizing problems, school problems and adaptive skills.
Helps differentiate between hyperactivity and attention problems.
Helps identify behavior problems as required by IDEA, and for developing FBAs, BIPs, and IEPs.
