All required forms for the Soccer Tournament 2026 are listed below.
Each form is completed and submitted online through Google Forms.
Teams and individual players must complete all required forms before they can be added to the tournament bracket.
IMPORTANT
Should your team win the Tyler Cup, please indicate whether your team is willing and available to represent the Diocese at the statewide tournament in Dallas on September 26–27. If yes, notify your team captain and review the details in the COMMUNIO DIOCESAN CUP tab under Purpose!

Team Registration & Tournament Rules Form

Purpose: This combined form officially registers your team for the tournament and confirms that the team captain has reviewed and agreed to all tournament rules. It includes team name, roster, captain contact, division, and acknowledgment of sportsmanship, game format, and referee authority.
Participant Waiver & Release of Liability
*Each player must fill their own form
Purpose: This form covers both the participant waiver and liability release. Each player acknowledges the risks of participating in a physical sport and agrees that the tournament organizers are not responsible for injuries, accidents, or stolen items. This keeps the event safe, friendly, and clearly understood by everyone.
