Transportation to Calvin Hall
Transportation to the conference venue (Calvin Hall) will be provided from Hotel Kabuki.

Please be sure to indicate during registration if you will require childcare services during the conference.
Accessibility
Accessible materials including large-print and braille programs will be available at the conference. Tactile maps are available of the neighborhoods surrounding the conference venue (Calvary Presbyterian Church) and the Kabuki hotel.
The conference Presentation Guidelines will ensure that oral presentations will be accessible to people with visual impairments.

From Hotel Kabuki to Calvin Hall
20-30 minute walk, traveling north on Fillmore is slightly uphill. All streets in this area have parked car lanes flanking both sides of the street, plus the lanes of moving traffic. Curb cuts and truncated dome mats are present at all four corners of every intersection.
Exit the main lobby of the Kabuki. In front of you is a car drop off/pick up point and a small parking lot with a rough-stone masonry border.
Turn right (north) and walk until you come to the sidewalk on Post Street. (About 85 ft).
Turn Left onto Post (west) and walk two blocks to Fillmore Street
The first street you will cross is Webster. It is a 4 lane street with traffic in both directions flanked by two parking lanes. In the center is a concrete median.
Fillmore Street: A two lane street with traffic in both directions flanked by two parking lanes.
Turn right on Fillmore street.
Travel north on Fillmore 8.5 blocks to Jackson Street.
Post Street: A two lane street with traffic in both directions flanked by two parking lanes.
The # 22 Bus travels north and south on Fillmore street and will drop you across the street from the conference venue. The first convenient bus stop is at Fillmore and Sutter. If you would like to board the #22 Fillmore (towards Bay Street) the bus shelter and waiting area can be found on the south-east corner before the crosswalk. Your stop will be at Fillmore at Jackson.
Sutter Street: A two-lane street with traffic in both directions flanked by two parking lanes.
Bush Street. It has three lanes of one-way, east bound traffic flanked by two parking lanes.
Pine Street. It has three lanes of one way, west bound traffic flanked by two parking lanes.
California Street. A four lane street with traffic in both directions flanked by two parking lanes
Sacramento Street. A two lane street with traffic in both directions flanked by two parking lanes..
Clay Street. A two lane street with traffic in both directions flanked by two parking lanes
Washington Street. Washington street from Fillmore west to Alta Plaza Park is a designated shared space, meaning the traffic patterns can change depending on the time of day. When traffic is allowed it is a two lane street with traffic in both directions flanked by two parking lanes.
Jackson Street- A two lane street with traffic in both directions flanked by two parking lanes.
The #22 Bus stops at the north east corner of Fillmore at Jackson directly across the street from the conference venue.
Calvary Presbyterian Church is a series of buildings on the north west corner of Fillmore and Jackson. The first taller sets of stairs interspersed with very wide columns is not the entrance. Beyond the older portion of the church there is another short 5 step flight of stairs that marks the main entrance. Ramp access is at the far right end of the complex, once up the ramp you will need to come left along an elevated portion of sidewalk to arrive at the main entrance doors.
From Calvin Hall to Hotel Kabuki
18-25 minute walk, mostly downhill on Fillmore. All streets in this area have parked car lanes flanking both sides of the street, plus the lanes of moving traffic. Curb cuts and truncated dome mats are present at all four corners of every intersection.
Calvary Presbyterian Church is a series of buildings on the north west corner of Fillmore and Jackson. Exit from the main exit down a 5 step flight of stairs and turn right.
Walk 8.5 blocks (south) on Fillmore
Jackson Street- Two lane street with traffic in both directions flanked by two parking lanes.
The #22 Bus travels north and south on Fillmore Street. The first convenient bus stop is at Fillmore and Jackson. If you would like to board the #22 Fillmore (towards UCSF Mission Bay) the bus shelter and waiting area can be found by crossing Jackson and walking about 100 feet down (south) on Fillmore. Your most expedient stop will be at the south west corner of Fillmore and Sutter. From there you will continue on foot to Post Street.
Continue down Fillmore Street (south)
Washington Street. Washington street from Fillmore west to Alta Plaza park is a designated shared space, meaning the traffic patterns can change depending on the time of day. When traffic is allowed it is a two lane street with traffic in both directions flanked by two parking lanes.
Clay Street. A two lane street with traffic in both directions flanked by two parking lanes.
Sacramento Street. A two lane street with traffic in both directions flanked by two parking lanes.

Turn Left on Post street (there is a Burger King at the north-west corner of Fillmore and Post) and travel two blocks to the Hotel Kabuki.
Webster. A 4 lane street with traffic in both directions flanked by two parking lanes. In the center is a concrete median.
After crossing Webster the hotel walkway to the main entrance is half a block up on the right.
The small Hotel Kabuki Parking lot is bordered by a rough-stone masonry wall that diminishes in height as you get closer to the pedestrian walkway. The pedestrian entrance is marked by a 1 foot by 2 foot section of stone wall with a metal japanese lantern style light fixture on top.
Turn right on the pedestrian walkway. The hotel lobby entrance will be on your left in 85 feet.

Code of Conduct & Safety Plan
The Smith-Kettlewell Eye Research Institute (SKERI) is dedicated to providing its employees and attendees of the Functional Vision and Accessibility (FVA) conference an environment that recognizes the inherent worth of every person and group and a safe and harassment-free experience. Harassment or hostile behavior are unwelcome, including speech that intimidates or interferes with a person’s participation or opportunity for participation. Harassment in any form, including but not limited to, harassment based on national origin, race, religion, sex, gender, or any other status protected by laws in which the conference or program is being held, will not be tolerated. Harassment includes the use of abusive or degrading language or gestures, intimidation, stalking, harassing photography or recording, inappropriate physical contact, and unwelcome sexual attention.
SKERI has a rigorous, clear, and widely advertised reporting process available to all conferees for confidentially reporting violations of our policy. FVA Conference Organizers and SKERI’s CEO, in conjunction with Legal Counsel (as appropriate), will review allegations of any such behavior on a case-by-case basis, and make inquiries of all those involved and any witnesses. The following are steps on how to confidentially report any alleged violations:
Alleged violations may be reported by calling our ethics hotline at 415-345-2033 which is a private and confidential line monitored by SKERI’s HR department.
When a report is made, it will be sent to the FVA Conference organizers immediately and will be reviewed with the SKERI CEO and, if appropriate, with legal counsel.
All persons involved and any witnesses will be interviewed and are port of the incident will be completed as expeditiously as possible by HR and conference organizers.
If it is determined that the code of conduct has been violated, at the very least the offender will be asked to leave and apologize to the complainant.
If the violation requires legal counsel or local authority involvement, any action required will be taken under the advisement of legal counsel.
Complainants are also free to contact local authorities if they wish by dialing 311. Anyone with harassment-related questions, concerns, or complaints is encouraged to contact the conference organizers using the resources above, and/or the HHS Office for Civil Rights (OCR). To file a civil rights complaint with HHS OCR, please consult their webpage on this topic. Please note: filing a complaint with SKERI or the conference organizers is not required before filing a complaint with HHS OCR, nor does it prohibit filing a complaint with HHS OCR. All allegations reported to SKERI’s HR department, and the resulting actions taken, will be documented.
