INCLUDED SHIRT ORDER FORM
Everyone should fill out the "2026 Production Shirt" form for their dancer! This form is for the shirt that is included with your production fee. If you are interested in ordering additional merchandise, please also fill out the "2026 Additional Merchandise Order Form" below in the next section.
ADDITIONAL MERCHANDISE ORDER
Payment for additional merchandise is due by September 15. Do not post payment to accounts. Please pay via cash or check made payable to SI Dance with your dancer's name and "merchandise" in the memo line
