Pick an appointment time frame available.  At this time you may add up to 2 to 3 additional patients per time slot with approval (please call or text 602-386-6339). The time slot is an approximation of the arrival time.  Since this is a mobile practice, arrival times can fluctuate.   

Cancellations should give as much notice as possible.  Cancellation after en route may be assessed a cancellation/travel fee.
