Here's a step-by-step guide on how to register for PhilHealth Konsulta if you do not have a PhilHealth number yet:
Visit the Local Health Insurance Office (LHIO) of PhilHealth in your area. You can find the nearest LHIO through the PhilHealth website or by asking for directions from your local government health center.
Bring a government-issued ID as proof of identification. This can be your passport, driver's license, or any other valid ID.
Fill out the PhilHealth Membership Registration Form (PMRF). The form can be downloaded from the PhilHealth website or obtained from the LHIO. Make sure to fill out all the required fields accurately and legibly.
Submit the completed PMRF to the LHIO and wait for your PhilHealth Identification Number (PIN). The PIN is a unique number that will serve as your PhilHealth membership ID.
