Registration Process

PhilHealth Konsulta is a program established by the Philippine Health Insurance Corporation (PhilHealth) that aims to provide affordable and accessible healthcare services to Filipinos, especially those who belong to marginalized groups. It is a primary care benefit package that covers medical consultations, diagnostic tests, and medicines for eligible individuals. K With Konsulta, PhilHealth hopes to improve the health outcomes and quality of life of Filipinos by providing them with the necessary healthcare services they need. 

Here's a step-by-step guide on how to register for PhilHealth Konsulta if you do not have a PhilHealth number yet: 

1. Visit the Local Health Insurance Office (LHIO) of PhilHealth in your area. You can find the nearest LHIO through the PhilHealth website or by asking for directions from your local government health center.

2. Bring a government-issued ID as proof of identification. This can be your passport, driver's license, or any other valid ID.

3. Fill out the PhilHealth Membership Registration Form (PMRF). The form can be downloaded from the PhilHealth website or obtained from the LHIO. Make sure to fill out all the required fields accurately and legibly.

4. Submit the completed PMRF to the LHIO and wait for your PhilHealth Identification Number (PIN). The PIN is a unique number that will serve as your PhilHealth membership ID.

5. Once you have your PIN, proceed to the Konsulta registration. You can register at any PhilHealth-accredited health facility, such as hospitals, clinics, and barangay health stations. Present your PIN and proof of eligibility, such as a senior citizen ID, disability ID, or certificate of indigency, and fill out the necessary forms.

6. Once your records have been updated, proceed to the Konsulta registration process.

Konsulta Registration Process:

The PhilHealth Konsulta process involves registering at the nearest Local Health Insurance Office (LHIO) or a Konsulta-accredited healthcare provider such as Rural Health Units and Barangay Health Centers. You may also register through the PhilHealth Member Portal.

To register, you need to fill up the PhilHealth Konsulta Registration Form (PKRF) for yourself and your dependents. Make sure to provide accurate and complete information.

After submitting your PKRF, you will receive a Konsulta Registration Slip with a QR code. This serves as proof of your registration and should be presented during your Konsulta consultation.
