Reimbursement instructions:

1. For those who have requested financial assistance in the registration form, you should have received an email with instructions on submitting your W-9/W-8 information. If you do not receive it (please check your spam folder), contact me at slou1@luc.edu. The email will contain a link where you MUST complete tax-related questions and submit the electronic W-9/W-8 form (Get your most recent I-94).

2. If you are NOT a U.S. citizen or permanent resident, please fill out the FNIF form.

3. Scan or take a picture of all of your receipts. All receipts must be itemized and include the payment information (i.e., the last four digits of your credit card). 

4. Send the following documents to Vicci (vbower@luc.edu) by the end of July, 2024. 

(a) Copy of your passport;

(b) Copy of your visa, I-94 (as it indicates on the FNIF first page), and FNIF form if you are NOT a U.S. citizen or permanent resident;

(c) All of your receipt;

(d) A cover letter.
