1. Must be a permanent/temporary/casual employee of Rizal Med with a minimum of three (3) years of continuous service. In case of Medical Officers III, minimum of two (2) years of continuous service.

2. Must have a performance rating of Very Satisfactory (4.0-4.99) or Outstanding (5.0) for the last rating period, at the time of the nomination.

3. Must not have been found guilty of any administrative offense, at the time of nomination.

A certificate of recognition and monetary incentives shall be given to each awardee.
