Preschool Modules

Welcome to the Preschool Modules! Before you view the first and last modules, you will need to complete a survey so we can learn more about you! 

Each Survey will take about 15-20 minutes and you will receive $25 for completing EACH!

To start the first module, click on the Parent Provides, Child Decides square, where it will take you to the page that allows you to begin the survey to open the first module. This survey should take between 15-20 minutes to complete (we do apologize for the length, but these are the questions that are required of us). Once you complete the survey, you will be directed to the first module. Once you've viewed that one, you can click the next button to view the second module "Make Mealtime Family Time." There is no survey with this one, we just need you to view the module. Once that's complete, you can click Next again, and you will be directed to the final module page "Hunger and Fullness." Again, you will take another 15–20-minute survey (it's the same one you took for the first module, but we're looking to see if the modules helped to alter any answers). Once you complete the survey, you can view the final module!

To thank you for the time it takes to complete this evaluation, you will receive a $25 Amazon gift card (via email) for completing the pre-survey, and then another $25 gift card for completing the post-survey. You do not need to complete all the modules at the same time, but you will need to view the first module immediately after completing the first survey. It will be the same thing for the last module; you'll need to view the module immediately after completing the survey. Unfortunately, there's no way to save your survey, so you'd have to complete it again in order to access those modules. 

After you complete the survey, you will be given access to the module.  When you're ready to view the first module, please click the "Next" button below.
