Children and youth under the age of 18 can be presented to the Coordinated Access Committee.  The committee serves as a liaison to local and regional services for children and youth with significant mental health concerns, addictions, serious behaviour challenges, significant developmental disabilities and/or a combination of complex needs. The application requires the consent of the parents/guardians and the outh (where age appropriate).

Funding requests on behalf of children and youth can be made using the attached form.  These requests often include recreation activities, camps, respite support and/or contributons to diagnositic assessments.  For more information, contact Mary Catherine Brisco (renfrewcccac@gmail.com)

Please ensure consent is completed for all funding requests for individual students.  The RCCCAC Application has the consent embedded in it so this form is not required if submitted the referral package.
