Fall Season

This website will be your primary source of information about the ultimate program at Robert Eagle Staff Middle School.  

• Fall Ultimate at Eagle Staff is an official Seattle Public Schools sport and governed by the policies of SPS, Eagle Staff Middle School and Disc NW. 

• SPS contracts with DiscNW to organize the games and create the game schedules.  

• The Eagle Staff Head Coach is hired by SPS and works with our volunteer administrator and the Athletic Coordinators at Eagle Staff to run our program. 

• Fall practices are twice a week on Tuesday and Friday directly after school until 5:30 on the Eagle Staff turf field. 

• Eagle Staff typically has 4 teams in the Fall (Varsity, JV, JVC, and a 6th grade team). 

• We do not know yet if we will have cuts. It usually depends on the number of Gx players who sign up and the number of volunteer coaches we can recruit. 

• Please note that completing the interest form and Final Forms does not guarantee a spot on a team. 

• Tryouts for the Fall season will take place on Friday 9/11, Tuesday 9/15 and Friday 9/18.  

• Tryouts will be held after school from 4:00-5:30 on the turf field at Eagle Staff. 

• All student-athletes must have the following items completed before they can attend tryouts:

  • Students must be registered with Ravens Ultimate Club through our interest form

  • Complete the SPS physical participation form by making an appointment with your preferred doctor or the health clinic at school.  

    • The history document (page 1) remains with your provider

    • The exam document (page 2) remains with your provider

    • The medical eligibility document (page 3) is uploaded through Final Forms.

  • Complete Final Forms for SPS authorization to play Ultimate Frisbee.  This includes checking Co-Ed ultimate frisbee and signing all waivers.

  • Students also sign all waivers on Final Forms.

Questions about Final Forms?

• If you are new to Final Forms or SPS.  Final Forms is the online athletic registration system.

• A parent/guardian must create a Final Forms account first before the student is able to create their account and sign forms. 

• All SPS families of students in grades 6-12 who are new to the platform received an email from Final Forms in August inviting them to complete their registration.  

• Students new to Final Forms will also receive an email to their SPS email address.

• Players must be enrolled with SPS at Eagle Staff Middle School. There are no "free agents" from other schools in the Fall. 

• All student-athletes must complete SPS FinalForms, which includes a signed physical form (good for two years.) Physical exams can be scheduled before, during, or after school at the Eagle Staff Health Clinic or at your preferred doctor's office. Once the physical form is filled out by a doctor, you will need to attach it to your student's FinalForm.  

• In the past we have not charged a player fee for the Fall Season. 

• Team jerseys are available for purchase or to loan. 

• An Interest Form (available in May) is required as well as FinalForms to receive more information about the Fall season.
