How do I make the right choice in a provider search?
Contact the provider in your region or county who are accepting new individuals that need supports. You can call and make an appointment to meet with them, or make an appointment to meet by phone. Explain that you are looking for an agency to provide service(s), and require about 20 minutes of their time and ask questions about their experience as a Case Manager and their agency track record.
· Where do I go to make a choice of a Case Management, Consultant or Supports Waiver CSC agency?
Answer = via the Primary Freedom of Choice / Waiver Change form or Case Management Change Form.
