Professional Case Coordination Services LLC continues to grow and provide quality supports to individuals with developmental differences.  We hire only well qualified caring and devoted persons dedicated to doing what is necessary for the individuals we serve. As we grow, we will be searching for qualified individuals to join our team.  Either you are looking for a change from where you work now in the field or currently working in the Case Management particular profession. We may be where you want to be. Qualifications are:

Masters or Bachelors degree in a social science. (preferred), or Associates Degree, or Social work degree or RN with a current license, or 6 years Case Management or Case Coordination experience in related field working with individuals with developmental differences or mental health. 

• Reliable transportation.   Must be willing to travel. 

• Must be able to pass full background check prior to working.  

• Must be a self starter and self motivated to complete the tasks timely and adhere to timeframes.  

• Must be comfortable with working from home with some in office meetings and or virtual meetings and conducting visits monthly with each person on the caseload.  

• Strict adherence with all DOH regulations/standards, Medicaid and other State entity regulations related to the care of an individual with developmental differences or mental health.  

• Computer skills a must. Must have own devices capable for Word, Excel, PDF etc.  (Chrome books are not advisable)

Specifics:

• Case Management tools required:

Computer, supplies, insurances, phone,  internet, reliable transportation, PPE etc.  all will be discussed in interview. (agency fax is available)

Current services provided by PCCS:

DD Waiver Case Management = DDW

Mi Via Waiver = MVW

(Pending in process) Guardianship.  

Specific trainings will be discussed at the interview.  

Applicant will be a sub-contractor.  Compensation depends on caseload.  

PCCS LLC. serves four of five large regions of New Mexico.
