You must provide a current certificate of immunization or a physical exam verifying protection against childhood diseases, as required by local, state, and other agencies. This certification can be obtained from the local health department or a private physician. Immunizations and exemptions have expiration dates, and updated certificates must be kept on file. Parents must provide a new certificate before the current one expires to avoid denial of admission or continued enrollment. If you choose not to immunize, a signed affidavit or medical exemption is required. During a vaccine-preventable disease outbreak, non-immunized children may be asked to stay home for safety reasons. The School Leadership Team reserves the right to assess enrollment for non-immunized children, in line with state laws.
The registering parent or guardian must notify the school in writing of all persons authorized to pick up their child. Anyone picking up the child must present a government-issued photo ID. The child can be released to anyone on the authorized pickup list without further notification. Changes to the authorized list must be submitted in writing. School staff cannot be listed as authorized persons unless they are related to the child, and minor children or siblings cannot be authorized for pickup.

Child paperwork requested and not returned within 30 days will also result in penalty fees. We reserve the right to request a letter from the physician stating that an appointment has been scheduled for X date in order for the student to continue attending school. This will be honored on a case by case basis after reviewing all pertinent information. The school reserves the right to disenroll a student at any time if documentation requirements are not met as these are state required regulations.
