The Arthur Child Comprehensive Cancer Centre will now function in Connect Care as it's own facility. This will impact some Patient Movement Workflows. Please familiarize yourself with the below workflows.
Once a decision to admit is made, the admitting service will become MRP upon transfer of care. Orders can be added to the ER encounter and then reconciled using the Interfacility transfer workflow.
The Admission medication reconciliation will need to be completed on the Arthur Child encounter.
From Arthur Child to FMC, or From FMC to Arthur Child and not returning
When a patient is being transferred from one hospital to another hospital location and is not expected to return, this is called an Interfacility Transfer (IFT). To facilitate this workflow, it is important to use the Interfacility Transfer Navigator.
In emergency situations e.g., Code Blue, when there isn't time to complete an IFT, Oncology providers may choose to enter suggested orders in the FMC ICU encounter for the physician to sign following the transfer.
From Arthur Child to FMC for procedure/diagnostic imaging, or from FMC to Arthur Child for radiation therapy
The Leave of Absence (LOA) workflow is used when a patient will temporarily leave their facility, typically for a procedure or diagnostic imaging.
Inpatient medications are not automatically accessible across encounters at separate facilities or where the outpatient department isn’t set up to be able to access inpatient medications within the same facility, such as Radiation Therapy department. To allow for inpatient medications to be accessible in these situations, it is necessary to use the Leave of Absence Navigator.
When a patient is getting Brachytherapy in the Arthur Child OR and is an inpatient at Foothills Medical Centre (FMC) or will be an inpatient post brachytherapy at Foothills Medical Center, a specific workflow needs to be followed so that the post op/admission orders are actionable at FMC.
To admit a patient from a hospital outpatient department(e.g., systemic treatment unit), enter an 'admit to inpatient' order on the encounter. Once released by the physician or nurse, the outpatient visit will be converted to an inpatient visit.
