This Class Registration Form has to be submitted along with the Laboratory Fee in order to hold a student's place in class. Go to the link below to download and print the form.  Once a student is registered for PREP classes and this form and supply fee are received, he/she is officially in the class!  As confirmation, an invitation by email will be sent to the student to join our Google classroom.
