The Patient Navigation Sustainability Assessment Tool (PNSAT)
Now that you have reviewed the background on the PNSAT and the sustainability planning process, you are ready to complete the assessment. As a reminder, the assessment is most informative when completed by at least 3-5 clinic staff in a variety of roles who are involved in the practice.
There are 2 versions of the PNSAT Available
Version 2 - PNSAT Short Version (2023) - applicable to all patient navigation practices
Version 1 - PNSAT for Preventive Cancer Screening (2019) - developed for clinic systems implementing cancer screening navigation

The PNSAT will take approximately 15-20 minutes to complete.
Upon completing the PNSAT, your score report will be emailed to you. If you do not receive your score report within 10 minutes, please check your spam/junk folder. Contact us if you need further assistance accessing your score report.
Optional: calculate the averaged scores of your full sustainability planning team. Use the scoring table on the last page of the PDF version of the tool.
Meet with your sustainability team to discuss the PNSAT results and develop a sustainability plan.
