Adapting the Wash U Sustainability Domains

Patient navigation practices generally encompass elements of clinical practices as well as public health programs. The Patient Navigation Sustainability Assessment Tool was developed to fuse the domains and questions from Washington University's Program Sustainability Assessment Tool and pilot version of the Clinical Sustainability Assessment Tool.
