Sustainability Planning Process

CCSP's Recommended Sustainability Planning Process

1. At least 3-5 individuals, the sustainability planning team, each complete the Patient Navigation Sustainability Assessment Tool (PNSAT) from their individual perspective. The assessment takes about 15-20 minutes to complete. Recommendations for roles to include in the sustainability team include patient navigator, operations/quality improvement, providers, administrators, leadership, finance, and other champions. 

2. Individual score reports will be emailed to respondents (if completed online) or can be calculated manually. Your team may choose to additionally calculate the overall average scores of the sustainability team to send to the group for use in discussion.

3. Develop a sustainability plan

a. Meet with sustainability team to discuss PNSAT results, including successes and areas for improvement.

b. Identify 1-2 PNSAT domain(s) that are feasible to address in a time-bound sustainability action plan. 

c. Outline sustainability action plan, including sustainability goal, partners to engage, resources need, steps, and timeline.

4. Implement the sustainability plan.

5. Re-evaluate sustainability capacity with the PNSAT at least annually, and update the sustainability plan as needed.
