1. Overnight Field Trip Permission Form (PGSD)

These are the standard Pottsgrove School District Field Trip & Medical Authorization form, the Overnight Field Trip Emergency Card, and Authorization for School Medication Administration. (Notarization not required)

2. *CBP Document of Permission *(Notarization required)

According to  US CBP (Customs and Border Protection), minors traveling with a group without both parents should send a notarized document signed by both parents/guardians granting permission to travel with authorized group leader(s). Parents/Guardians seeking a notarized travel authorization MUST appear in person with their complete but UNSIGNED document of permission. Parents/Guardians should only sign their document when directed to do so by the notary.  Notary services can be found at the Pottstown Library (~$5). OR you may be able to secure notary services at no cost through Chaydeanne Kleinfelter by making an appointment with her at the district office (office hours only)

NOTE: Notaries are legally barred from notarizing documents not signed in their presence. If you accidentally sign ahead of time, get/print a replacement document.
