**Please note that 30-40 total registration spots are available per workshop (site-dependent).          Student registration is capped at 10-15 spots for each workshop**

REGISTRATION INSTRUCTIONS & POLICIES:

Complete the registration form below.  You will receive an email from anatomy.canada.care@utoronto.ca to confirm that you have been accepted into the workshop or placed on the waiting list.  Please check spam if you haven't received this email within 1-2 days of registration submission.

Once you receive the confirmation email, you must submit registration fee within 24 hours in order to maintain your registration spot.  Spots are limited so early registration is encouraged.

A minimum registration of  15-20 (site dependent) is required for the workshop to be conducted.  The organizers of each workshop reserve the right to cancel any workshop which is under-registered.  This decision will be confirmed 5-7 days prior to the workshop to all participants.

Cancellation policies:  

Should the workshop be cancelled by organizers, all participants will receive a full refund.

Cancellation by participants must be received in writing to:  anatomy.canada.care@utoronto.ca 

• Should cancellation notification be provided 7 days or more prior to the workshop, a $100 processing fee will be withheld and the balance refunded.

• Cancellations 7 days or less of the workshop date will not be refunded.

Non-attendance to workshop:  no refund.

Transfer of registration, in the case of cancellation is permitted but must be submitted in writing and approved by the organizers prior to the workshop.  Substitute registrants is not permitted the day of the workshop.
