Here are the instructions on how to properly facilitate the Client Satisfaction Feedback (CSF) Forms:
Click here to download the ARTA-compliant PCA CSF Form available in both English and Tagalog versions. This feedback form must be used to collect the client satisfaction data.
The following PCA offices listed below must appoint a dedicated Client Satisfaction Measurement Officer (CSMO) tasked with guiding and informing clients about the completion of the feedback form.
The PCA services (internal and external) are listed here.
CSMOs are required to input the data from filled-out CSF Forms onto the designated online platform - bit.ly/pca-feedbackform - every Friday.
Additionally, CSMOs are responsible for securely storing the filled-up CSF Forms within their respective offices.
Only the data and information encoded by authorized Email addresses listed below will be recorded in the final Client Satisfaction Measurement Report.
