Activity Consent Form - Required for BSA aviation flights and recommended for all activities.

Annual Health and Medical Record - All participants in all Scouting activities complete Part A and Part B. Give the completed forms to your den leader. This applies to all activities, day camps, local tours, and weekend camping trips less than 72 hours. Update at least annually.

Family Talent Survey - This form will assist your Den Leader and pack leadership in knowing your family’s interests. This will help in planning of den and pack activities.

Guide to Safe Scouting - All participants in official Scouting activities should become familiar with the Guide to Safe Scouting and applicable program literature or manuals, and be aware of state or local government regulations that supersede Boy Scouts of America practices, policies, and guidelines. The Guide to Safe Scouting is an overview of Scouting policies and procedures gleaned from a variety of sources. For some items, the policy statements are complete. Unit leaders are expected to review the additional reference material cited prior to conducting such activities.

Pack Overnight Campout Appraisal Form - Pack 654 conducts overnight campouts.  The location is a site that is approved by the local council using the Pack Overnight Campout Appraisal Form.
