To ensure we can continue providing individualized services, the below patient acuity level guideline was created. Please note, this is simply serving as a 'guideline' and not an official document. Regarding the assigning of patient acuity, there is flexibility within the three (3) levels.
For example--a client with diabetes may experience a hospitalization due to blood sugar issues. Upon discharge, they might require additional services and temporarily move from Level 3 to Level 2. Once stabilized, they would return to Level 3.
As mentioned previously, teams were initially based on 125 clients. However, because of the levels, we were able to manage up to 160 clients per team, depending on the distribution across levels. It is the responsibility of team coordinators to ensure that no single team is composed of clients from only one level.
