Key Points

• Ensure that plans of care include a goal and intervention that is measurable and time specific, or documented in case notes.

• Case notes must reflect the plan of care and be detailed.

• Ensure that a plan of care is completed upon intake for each patient; it is imperative that each patient has a plan of care & that staff work with each client on their POC goal throughout treatment.
