You will be redirected to the Google Form where you will complete and submit your written & video application.
Follow all instructions provided in the form carefully. You will be asked to provide the following information:
Personal Information: Your full name, email address, and current enrolled high school.
Parent/Guardian Contact Information: The full name, phone number, and email address of your parent or guardian (required for communication and liability purposes).
Academic Information: Your current GPA (both high school and community college, if applicable), a list of relevant coursework, and a brief description of your academic standing.
FHS Program Acknowledgements: You will be required to acknowledge that you have read and agree to our guidelines as listed on the application.
Once you complete the written section of the application, you will be required to submit a 5-7 minute video that will be reviewed by program staff. See the instructions to make and submit your video below!

Step-by-Step: How to Make & Submit Your Video
Prepare your setup. You should be in a quiet, well-lit space (face a window or lamp). Make sure your phone/tablet/laptop is at eye level, and landscape/horizontal. Make sure your background is clean; dress neat (business-casual).
Do a quick tech check (make sure your technology works and takes a good quality video). Record a test clip; play it back to confirm clear audio and bright image. If audio is low, move closer or reduce background noise.
Record your video (aim for a minimum of 5 minutes and no more than a maximum of 7 minutes). Start with your name, grade, and school. Make sure you speak clearly; keep energy up; glance at cues—not a script. Don't be afraid to stumble or mess up in your video; we are not looking for perfection! However, make sure your video represents you in the best way possible before submitting your application!
Save your video to your device. You will need to do this in order to upload your video to YouTube or Google Drive.
Choose how you’ll submit your video. You can either submit your video as a Link via YouTube or upload your video file to Google Drive and submit your google drive link to the application. See more detailed instructions on how to submit your video below.
If you would like to use a YouTube link:
Go to https://www.youtube.com --> Sign in using your google account --> Click the "Create" button at the top right corner --> Select "Upload video" --> Upload your video file (make sure it is the right format) --> Enter your video details as described below --> Click "Continue" --> Continue past all additional video elements (subtitles, cards, etc.) --> When you reach the "Visibility" page click "Unlisted" (this allows anybody only with the video link to watch your video) --> Click "Save" --> Copy the link that YouTube provides to you and paste it into the application portal (google form).

Playlist: Ignore this section.
Audience: Select "No, it is not made for kids." This allows us as reviewers to view the video freely without any COPPA (Children Online Privacy Protection Act). Selecting this does not indicate your video is inappropriate in any way, it is simply to make access easier on our end.
Age-Restriction: Select "No, don't restrict my video to viewers over 18 only."
If you'd like to use a Google Drive link:
Record your video --> Save it as a file in your computer's desktop --> Go to google drive and sign in to your google account --> Select "New" at the top left --> Select "File upload" and select your video file saved to your computer --> Navigate to your uploaded file on google drive (make sure you know which folder it is located in) --> At the top right, click on "Share" --> Change "Restricted" to "Anyone with the link" --> Click "Done" and copy the google drive link provided to you --> Paste your google drive link in the application portal.
It is imperative that you change access to your video to "anyone with the link". Otherwise, we will not be able to watch your video.
After submission, your application will undergo a comprehensive review by the FHS Program admissions committee. Each applicant will be scored holistically (i.e. not one section in the application carries more weight than another).
How We Evaluate Your Application
Admissions decisions are based on a holistic review—meaning we look at both who you are and what potential you show. We will evaluate your application based on:
Your “WHY” for pursuing healthcare AND applying to the FHS program.
Your commitment to learning and motivation.
Your ability to communicate clearly, with authenticity, and professionally
Applicants with the highest scores may be invited to participate in an interview conducted by FHS program staff and faculty members.
The Future Healthcare Scholars (FHS) Program seeks students who demonstrate a genuine curiosity and passion for the health sciences and healthcare fields. Your application should reflect who you are, what motivates you, and how this program aligns with your goals.
Show your authentic interest. The admissions team values sincerity over formality. Be honest about what sparked your interest in healthcare and what keeps you motivated to pursue it.
Quality over quantity. The number of extracurricular activities or opportunities you talk about does not influence your application outcome. Instead, if you choose to talk about prior experience, focus on the depth of your involvement—highlight experiences that have been meaningful to you and have inspired or sparked your interest in healthcare.
Be reflective. Use your video application to tell your story. Explain why healthcare interests you, how your experiences have shaped your perspective, and what you hope to gain from participating in the FHS program.
No prior experience required. Prior exposure to healthcare—such as research, shadowing, or internships—is not required and not used as a selection criterion. The program is designed to introduce and inspire future scholars at all levels of experience.
Remember that we do not consider prior healthcare exposure or experience when scoring your application, but if you feel that talking about an experience will better define you as a person, definitely feel free to do so!
