Patient received therapeutic stretching in conjunction with --- Skin assessment completed regularly throughout SWD application to insure proper thermal response. Upon completion of session skin was intact and in normal condition.
ESTIM STROKE – OMNISTIM – Pain- TENS LVPC Motor Protocol x 15mins; ELECTRODE PLACEMENT: ((CHANNEL A)) negative electrode: body inches proximal to the superior border of the patella at the bulge of the Vastus Medialis muscle, Positive electrode: In the depression anterior and inferior to the head of the fibula; ((CHANNEL B)) negative electrode: 3 body inches above the lateral malleolus between the posterior border of the fibula and the peroneus longus and brevis tendons, positive electrode on the dorsum of the foot, distal to the junction of the first and second metatarsal bones.
Patient received eledtro acupuncture using TENS- LVP motor to SP-10, GB-34, ST-36, GB-39, LV-3: Acupoints to facilitate neuro reeducation, decrease tone to LE and for stroke recovery.
E-Stim applied to left foot, left ankle and left knee in order to facilitate muscle reeducation, facilitate neuromuscular reeducation and enhance muscle strength, power and functional activity tolerance with intensity level, durations and settings at Neuro Re-Ed TENS LVPC Motor. In order to improve bed mobility and hygiene, patient was instructed through bridging and rolling to R and L exercises in supine for improved sequencing and tolerance to activities. Patient required min A and maximal verbal and tactile cueing for correct performance of tasks. Patient was coached through gait exercise x 100 feet using R/w to increase tolerance to activity for improved functional mobility. Patient required min A to come to standing and CGA during ambulation with assistance with guiding walker around curves. Patient’s BP was 110/73 mmHg and HR was 56 bpm in sitting before ambulation. After ambulation exercise, patient’s BP was 102/62 mmHg and HR was 62 bpm.

E-Stim applied to left knee and right knee in order to facilitate muscle reeducation, stimulate muscle contraction and facilitate neuromuscular reeducation with intensity level, durations and settings at Functional Re-Ed Slow Cycle Fast Cycle; AA muscle contraction.
E-Stim applied to left knee and right knee in order to stimulate innervated muscles to cause a muscular contraction to strengthen and assist in functional activities, facilitate muscle reeducation, facilitate neuromuscular reeducation and enhance functional mobility with intensity level, durations and settings at Functional Re-ed Slow Walk - AA muscle contraction.
E-Stim applied to left ankle and left knee in order to stimulate innervated muscles to cause a muscular contraction to strengthen and assist in functional activities and facilitate neuromuscular reeducation with intensity level, durations and settings at Neuro Re-Ed TENS LVPC Motor.
