Umpires must provide a written acknowledgement to the AFL that match payments should not be taxed. This is done by submitting a Statement by Supplier Form. Please upload a photo or PDF of your completed Statement by Supplier Form to your OHQ Profile if you haven’t done so already. In OHQ go to [The Avatar top right of the screen > Account > Appointments> Accreditations > Select the “+” button > Follow the Prompts]
When completing the form select the following box “The supplier is an individual and has given the payer a written statement to the effect that the supply is either: made in the course or furtherance of an activity done as a private recreational pursuit or hobby”

All umpires over the age of 15 must provide a valid working with children check number and upload a photo of their Ochre Card (or valid interstate WWCC Card for visiting umpires).
In OHQ go to [The Avatar top right of the screen > Account > Work With Children> Select the “+” button > Follow the Prompts]
