Below is the Physician Release Form for Diocesan sports. This document must be printed out, completed, and returned to the school office before a student is permitted to attend any sports practices or games for NCA Academy. The physician release form must be signed by a doctor following a physical exam. Please turn completed forms in to the school office in an envelope marked "C/O Athletic Director, Sports Registration". PLEASE DO NOT SUBMIT THESE FORMS TO YOUR CHILD'S COACH. They must be turned into the school office.
