Final Forms Registration Required

Northridge Middle School will use the same registration program that Northridge High School will use called Final Forms.  Students wishing to participate in sports at Northridge Middle School need to make sure that they meet all the requirements necessary to begin practice. A physical examination, insurance, sudden cardiac arrest, and concussion form must be on file in the Northridge office via Final Forms before anyone may participate in any practice.  The physical must have taken place and be dated after April 2, 2023 to be valid.  All the required forms are available through Final Forms.  

To register your child for any sport, you must create an account in Final Forms.  This account will follow your child from 6th grade through 12th grade.  You need the following to create an account:

Please take time to complete all forms required and SELECT sports they are even considering trying out for.  Checking a sport does not commit them to trying out for the sport.  You will complete all parts of the physical form except the doctor section via Final Forms, then print and take to your physician.  Once your physician has completed the necessary sections, you will be required to scan (or take a picture) and upload to Final FormsPaper copies will NOT be kept at the NMS Office!  Please contact Mr. Miller at millers4@mcsin-k12.org with questions.

Only the IHSAA physical form will be accepted. 

Physicals must be uploaded to Final Forms before the first day of practice.

If we do not have a physical on file, athletes will only be able to watch.  We strongly encourage you to upload all forms before July 26, 2023.  The school office will be open during the following office hours 2023 for additional help:

July 18 - July 20        7:30 am - 2:30 pm

July 24 - July 27        7:30 am - 11:00 am
