If you are a parent, volunteer, or other person that needs a vehicle inspection so you can drive on a field trip, this is the form you need to complete and return to the NMHS main office.

All requests for official transcripts must be completed online and will be sent electronically.
Current NMHS students: $5 per official transcript
Alumni: $10 per official transcript
Fee waivers available upon request for qualifying students.
