Before participating in any team practice or activity:

• All swimmers and divers must register at NHS FinalForms.

• All athletic participants must complete a PREPARTICIPATION PHYSICAL EVALUATION FORM (PPE Medical, valid for 2 years) with a physician's signature and upload it to FinalForms, or send it to Mr. Brakke, Athletics & Activities Director's email: brakkeb@bsd405.org

• All divers must also take the ImPACT Test (valid for 2 years) before participating in their first competion.  

• International & Big Picture school athelets, please contact Ms. Poad at poadp@bsd405.org
