All CHW services are provided at no cost!

Community Health Workers (CHWs) act as a connection between people in need and community resources that can help. CHWs have a wealth of knowledge and can act as advocates when people need them the most.


There is no single set of activities that CHWs can do. When a CHW meets with a participant, they work together to identify needs and choose goals to work toward.


Some common examples of resources that CHWs assist with include:

  • Pre- and post-natal education and support including supplies for mom and baby

  • Obtaining medical insurance

  • Obtaining IDs

  • Employment and housing

  • Transportation to medical and WIC appointments

  • Food and clothing resources

  • Linking to Mental Health Providers and Support


But CHWs can do so much more! Each person has their own story and situation. CHWs will meet with each participant personally to evaluate needs and attempt to find resources to support them.
