After the operation of Porta Cath insertion:
Complete the PWH Oncology CVAD Usage Record.
Fax to 3505 4711, confirm the fax by 3505 1074.
File the faxed form.
Surgical IC/ Delegate acknowledge receipt of the ordered goods and fax to confirm.
**Please keep the faxed PWH Oncology CVAD Usage Record in the file. (within the plastic box of the Porta Cath)**
