In order for your cadet to participate in any NJROTC activity or varsity sport in the state of Florida, your cadet must have a CURRENT annual sports physical from a qualified medical practitioner - NO EXCEPTIONS! This is a safety issue for your cadet given we do rigorous exercises and physical training that can be very challenging to the body. Occasionally, there seems to be some confusion about this process that this page will clarify:
a sports physical in Santa Rosa County is only good for ONE YEAR FROM THE DATE OF THE MEDICAL EXAMINATION and must be updated annually.
a cadet cannot participate in NJROTC without a current sports physical since we do Physical Training (PT) weekly and Physical Readiness Evaluations. This is explained in the Regulations for Citizenship Development (NSTCINST M-5761.1B). Again - it is a risk management procedure to make sure your cadet has no limitations that could possibly result in injury or death from rigorous/strenuous exercise.
The Navy's Citizenship Development Management Information System (CDMIS) will not permit us to enter cadet fitness scores without a current physical and is a "hit" on our annual statutory Area Manager's Inspection if a cadet is delinquent.
a cadet cannot participate in our quarterly Physical Readiness Evaluation without a complete clearance in AthleticClearance.com and will receive a ZERO for that graded event without a clearance.
a cadet cannot go on NJROTC field trips, overnighters, competitions without a current sports physical - this includes STAFF TRIP!
a cadet cannot participate in summer camps (BLT/LA/GA/SLS/NS-1 Camp) without a current sports physical - this includes NS1 Orientation Camp as cadet staff OR new NS1.
ONLY the FHSAA EL2 Physical Form is authorized/acceptable for the physical (downloadable below).
The EL2 is comprised of five pages (see the form below). The first three pages are to be completed by the cadet/parent for the health care provider and are to be kept by them, or the parent/guardian. Page four needs to be uploaded into the AthleticClearance.com system. We highly recommend you keep the original for your records should the AthleticClearance.com system not work properly! The unnumbered page, after page four, is the supplemental page - it's a referral page to be utilized if your cadet has an injury/condition that needs a referral.
The school district is also requiring ALL STUDENTS who participate in competitive physical activities have proof of insurance and to consent to drug screening. This form MUST be notarized! We have included it in the packet (last page) and you must upload it into AthleticClearance.com.
Cadets with limitations are approved to participate in NJROTC as long as limitations are annotated on a complete/current sports physical.
For "Sport" that your cadet will be playing, please say: "ROTC" (and whatever other sport if they are doing varsity athletics).

All student/parent information is complete on all pages to be uploaded into AthelticClearance.com (page 4, supplemental if required, notary page).
INSURANCE COVERAGE IS REQUIRED - NO EXCEPTIONS! Navarre High School has information for providing very affordable insurance for your student/cadet. Please contact the front office (850-936-6080) for more information if needed.
All signature lines for students (cadets) have been signed and dated.
All signature lines for parents of students (cadets) have been signed and dated .
**BUT... THERE IS ONE PARENT SIGNATURE THAT MUST BE SIGNED AND DATED IN FRONT OF A NOTARY PUBLIC!**
All signature lines of the notary box MUST be signed by a parent/guardian in front of a Notary Public - NO EXCEPTIONS! (yes - this is repetitive because it is often forgotten). We have several Notary Publics at the high school - check with the front office.
There must be a signature and date by the medical practitioner who conducted the medical evaluation. It must be legible - NO EXCEPTIONS!
Please start this process as soon as possible - DO NOT DELAY! Please help one another! Ask other parents for assistance if needed.
