• Teachers are sent referral forms at the start of every quarter

• You may complete the group referral form below at any time requesting your child be included in the next quarter's group enrollment

*Groups are formed based on the most pressing student needs, group availability, and parent permission. Not every group will be held every quarter, and due to student referral/parent permission, there may not be enough students to form a group. If you request that your student/child be included in a group and the group is not available in that quarter, they will be added to my individual caseload to be seen on a weekly basis until an appropriate group spot opens up.
