CLINIC EXPECTATIONS & MILESTONES
Arrive on-time (8:45 AM sessions, 1:30 PM session).
Arrive prepared to discuss your patients (i.e. thorough pre-rounding).
Arrive in professional attire.
Complete all paperwork assigned.
If you do not have time, please return to the preceptor.
Check in with preceptor before leaving.
Follow through on all patient-related tasks as discussed with your preceptor.
Be respectful to all staff/members of patient care team.
Prioritize multiple competing demands to complete tasks in a timely and effective manner.
Reconcile patient medication list at every encounter.
Communicate plans with patient effectively.
Employ methods such as teach-back, motivational interviewing, etc.
Communicate with staff actively by utilizing the status bar.
PROBLEM-BASED LEARNING & IMPROVEMENT
Elicit feedback from every attending you work with during rotation.
Seek help when you need it!
Actively participate in quality improvement projects in the clinic.
Follow-up on results and outcomes of your patients.
Identify your practice team and ensure proper transition of care when needed.
Enlist the help of your multidisciplinary team (e.g. high risk case managers, behavioral health, social work, diabetic educators, nursing team).
Identify social determinants of health impacting your patients and provide appropriate resources to mitigate these factors.
Apply cost-awareness to patient's management plans.
Ensure proper documentation of medical record to meet requirements for quality indicators and patient safety.
