To ensure your safety, we are offering Telehealth as a means to conduct appointments.

Please review the Telehealth Consent form and the Telehealth signature page. Fill out the information and email back to us with a copy of your Driver's License.

Once we receive the forms we will schedule an appointment and call you with the appointment time.

If you are not able to fill out the form, please reply to the email saying "I have read the consent form, I agree and give permission to Monroe Pediatrics to give medical treatment to my (name of patient) via Telehealth."
