Dementia:
Dementia is not a specific disease but is rather a general term for the impaired ability to remember, think, or make decisions that interferes with doing everyday activities. Alzheimer’s disease is the most common type of dementia. Though dementia mostly affects older adults, it is not a part of normal aging.
Alzheimer's:
Alzheimer's disease is a chronic brain disease that gradually erodes an individual’s memory, intellectual abilities and personality.
Consultation:
A free fifteen-minute meeting with Heather to learn what your specific care needs are and to create a person-centered plan to best support you along your dementia/caregiving journey. A one-to-two-hour appointment will be scheduled with you within two weeks of your initial consultation. Follow-up appointments will be determined upon our first appointment.
Care Recipient:
The individual in whom the services are for (typically the individual living with memory loss, Alzheimer's, or other related dementia(s)).
Caregiver/ Care Partner:
A family member, friend, or neighbor or paid helper who regularly looks after or supports a person who is sick, elderly, or disabled.
Person-Centered:
Person-centered planning starts with discovering what is important to and for YOU. We want to know what is most important to you to help support your journey.
Person-centered planning starts with listening and respecting what you say. We want to know what your goals are, what's important to you, what are you good at, what are your dreams for the future, do you like what your day looks like, what you would like your day to look like, if you like where you live or if you wish to live elsewhere, if you would like someone to provide care for you, or more assistance in other ways, etc.
