The internship agreement is composed of 7 parts on the first page and it should look like above picture

Part 1: the university or the school at which you are enrolled.

• If you are enrolled at Université Paris-Sud, this part should be already filled out if you have the proper agreement version.
• If you are enrolled as a student in another school or university, you should fill out this part with your school or university.

Part 2: the university in charge of the master (Université Paris-Sud).

• This part should be already filled out.

Part 3: the hosting company or lab

• Ask the hosting company or lab to fill out this part.

Part 4: this is your part.

• ''INE No. / N° INE'': this is the French national student number. If you are not French, I think that you should have received this number when you registered to the University. You can probably check that with Murielle.
• ''Title of the degree program taken / Intitulé de la formation suivie'': I already put Master.
• ''Program / Mention'': put Informatique.
• ''Training element / Elément de formation'': M2 Interaction - Human Computer Interaction or M2 HCID for HCID students.
• ''Annual hourly training volume (outside of the internship) / Volume horaire annuel de la formation (hors période de stage)'': put 294h.
• ''Status of the student / Statut de l’étudiant'': checked student in first-degree course or étudiant en formation initiale (French version).
• ''Social Security affiliation organization / Organisme d’affiliation de sécurité sociale'': put the name of your health insurance.

Part 5: the academic referent.

• Once you have submit your request internship validation form, you can check this spreadsheet to know your an academic referent.
• Fill out the information of your referent (contact information is available in the spreadsheet).

Part 6: your supervisor in the hosting company or lab (i.e. internship tutor).

• Ask your supervisor in the hosting company or lab to fill out this part.

• Fill out this part with the hosting company or lab.
• I already filled out two items:
  • ''Monitoring by the instructor / Suivi par l’enseignant'': I checked the 1st and 2nd boxes.
  • ''Evaluation procedures / Modalités d’évaluation'': I checked both boxes.
• ''Competencies to be acquired / Compétences à acquérir'' has to be filled out with the hosting company or lab, but here is a suggestion:
  • English: Become autonomous in the design process of interactive systems or interfaces, participate in their implementation and be able to evaluate their quality in a scientific and rigorous way.
  • French: devenir autonome dans le processus de conception de systèmes interactifs ou d'interfaces, prendre part à leur implémentation et être capable d'évaluer de façon scientifique et rigoureuse leur qualité.
• ''Activities assigned / Activités confiées'' has to be filled out by the hosting company or lab.
