This is a narrative assessment report I completed for my Cognitive-Linguistic Disorders course. This assignment aided my cognitive-communicative assessment skills through the evaluation of a simulated patient with a history of traumatic brain injury (TBI) and resected glioblastoma. I had the opportunity to administer various assessment tools like the Functional Assessment of Verbal Reasoning and Executive Strategies (FAVRES) (Macdonald, 2005) and the Cognitive Linguistic Quick Test-Plus (CLQT+) (Helm-Estabrooks, 2001). Multiple assessment measures were utilized to identify potential areas of improvement for the promotion of functional independent living. This was a home-based assessment that supported my learning of the SLP role across the continuum of care and emphasized the importance of clearly outlining the patient's personal goals.

This is a progress note for a patient I worked with at Westland House who presented with cognitive deficits in memory, attention, and problem-solving. The session focused on memory through a two-step written instruction task and the training of external memory strategies. I collaborated with the patient and a member of her support system about ideas to overcome her memory deficits upon discharge. I recommended the patient's family receive appointment reminders and encouraged effective communication across involved parties. I also suggested developing a folder containing pertinent medical information and a calendar with upcoming appointments. Cognitive interventions should encourage active involvement, provide education, and train functional strategies to support an individual's needs (McCullough, 2014). This session highlighted the importance of functional cognitive intervention, family involvement, and creative solutions to overcome the patient's challenges.

This is an initial assessment report and treatment material for my first adult client, whom I worked with at the Adult Language Clinic. The client's initial and progress reports are featured in the 612 clinical experiences page. I administered the Montreal Cognitive Assessment (MoCA) (Nasreddine et al., 2005) and subtests from the Rivermead Behavioral Memory Test- Third Edition (RBMT-3) (Wilson et al., 1989) to gain greater insight into this client's reported attention and memory impairments. The patient exhibited deficits in visuospatial skills, executive function, language, abstraction, and memory. I created a goal to target this patient's memory deficits through the use of external and internal memory strategies. Internal and external memory strategies have been shown to improve memory outcomes and support quality of life (Davis et al., 2019; Fleck & Corwin, 2013). The client demonstrated increased awareness and independence to utilize external visual aids and his AAC device to support his memory deficits in treatment sessions. I learned a lot from this client about the overlap between aphasia and cognitive impairments post-stroke and the importance of trialing various treatment approaches to support optimal outcomes.
