I do many classroom assessments (Patricia Cross) to gauge student learning throughout the semester. These help me see where students are not understanding and help me lead them to resources that might be helpful to review or practice. I usually use Google Forms for this.
For my online Technology in the Classroom course, I do a pre-course survey. I utilize this to identify students who are very early in their coursework, have a focus on younger children so that some of the technology is not readily applicable to early elementary classrooms, find out how they feel about technology, survey their knowledge of educational technology tools. I then create a plan that moves them forward. Here is the form I use to collect the data:

RESULTS:
This analysis of the initial survey takes a bit of time to go through all the students and identify individualized instructional activities, but it provides for a more engaging class and helps identify students at risk early in the course for better student success!
