Insurance and Fees

I am an out of network provider, meaning that you are responsible for all payments, and can submit to your insurance provider for out-of-network reimbursement. In order to better understand your out-of-network benefits, the following questions are helpful to ask your insurance provider:

• Do I have out-of-network benefits for behavioral health?

• Do I have a deductible, and if so, how much is it and how much has been met?

• Do I require prior authorization for the following CPT codes?

90791, 90834, 90832, 90846, 90847, 96136, 96137, 96132, 96133

• Are the above listed codes reimbursable and at what rate will they be reimbursed?

• Do I have a copay or coinsurance for out-of-network services?

• Are there limitations to the above listed codes?

• Does the provider need to provide any specific forms or documentation?

• Where do I submit documentation for reimbursement?
