Effects of Nordic walking training on quality of life, balance and functional mobility in elderly: A randomized clinical trialPurpose There is physiological and biomechanical evidence suggesting a possible advantage of using poles in walking training programs. The purpose of this proof-of-concept study was to test the hypothesis that untrained elderly training Nordic walking for eight weeks will show higher improvements on the functional mobility, quality of life and postural balance than that training without poles; more likely to occur in self-selected walking speed (primary outcome), and the locomotor rehabilitation index than the quality of life, the static balance and the dynamic stability. It was a two-arm randomized sample- and load-controlled study. Methods Thirty-three untrained older people were randomly assigned into Nordic walking (n = 16, age: 64.6±4.1 years old) and free walking (n = 17, age: 68.6±3.9 years old) training groups. Results Improvements in the self-selected walking speed (primary outcome, p = 0.011, ES = 0.42 95%CI -0.31 to 1.16), locomotor rehabilitation index (p = 0.013, ES = 0.36; (95%CI -0.39 to 1.10), quality of life (p<0.05), static balance (p<0.05) and dynamic variability (p<0.05) were found in both groups. Conclusions The hypothesis was not supported, our findings indicated that after 8 weeks, the Nordic walking training did not result in greater improvements than free walking training for the primary outcome (self-selected walking speed) and most of the secondary outcomes (including locomotor rehabilitation index, static balance, dynamic stability, and psychological and social participation domains of quality of life). A systematic review of the effects of strength training in patients with fibromyalgia: clinical outcomes and design considerationsFibromyalgia (FM) is characterized by chronic and generalized musculoskeletal pain. There is currently no cure for FM, but palliative treatments are available. One type of treatment is strength training (ST). However, there is a need for more information on optimal training protocols, intensity, and volume needed to improve symptoms. The aim of this study was to analyze the effects of ST in the treatment of FM through a systematic review of experimental research. Medical Subject Headings search terms and electronic databases including Scientific Electronic Library Online, PubMed, Science Direct, Web of Science, and Physiotherapy Evidence Database were used to identify studies. The inclusion criteria were met by 22 eligible studies. Most of the studies were conducted in the United States (36%), Finland (23%), Brazil (18%), and Sweden (18%). The studies showed that ST reduces the number of tender points, fatigue, depression, and anxiety, and improves sleep quality and quality of life in patients with FM. The intervention period ranged from 3 to 21&nbsp;weeks, with sessions performed 2 times a week in 81.81% of the studies, at initial intensities of 40% of 1-repetition maximum. The repetitions ranged from 4 to 20, with no specific protocol defined for ST in FM. The main results included reduction in pain, fatigue, number of tender points, depression, and anxiety, with increased functional capacity and quality of life. Locomotion as a Powerful Model to Study Integrative Physiology: Efficiency, Economy, and Power RelationshipLocomotion is the most common form of movement in nature. Its study allows analysis of interactions between muscle functions (motor) and lever system arrangements (transmission), thereby facilitating performance analysis of various body organs and systems. Thus, it is a powerful model to study various aspects of integrative physiology. The results of this model can be applied in understanding body functions and design principles as performance outputs of interest for medical and biological sciences. The overall efficiency during locomotion is an example of an integrative parameter, which results from the ratio between mechanical output and metabolic input. Although the concepts of cost (i.e., metabolic expenditure relative to distance) and power (i.e., metabolic expenditure relative to time) are included in its calculation, the overall efficiency establishes peculiar relations with these variables. For a better approach to these aspects, in this study, we presented the physical-mathematical formulation of efficiency, as well as its conceptual definitions and applications. Furthermore, the concepts of efficiency, cost, and power are discussed from the biological and medical perspectives. Terrestrial locomotion is a powerful model to study integrative physiology in humans, because by analyzing the mechanical and metabolic determinants, we may verify the efficiency and economy relationship through locomotion type, and its characteristics and restrictions.
